Even with evidence of regression and the absence of detectable DNA, it is still possible for the lesion to reactivate, providing further evidence that the development of cervical neoplasia is causally related to the presence of high-risk HPV. The possibility of cure of CIN without radical or ablative treatment is based upon the spontaneous regression rate for CIN 2 to 43% and 32% for CIN 3 within 2 years, although this may still be associated with persistent HPV infection and the disease is still likely to recur. (9) Current guidelines and management of HSIL are based on the idea that these are precursors of invasive cancer. As the disease has the potential for significant morbidity and mortality, swift and decisive treatment is often recommended. However, the significant emotional and physical cost of treatment for a disease which may still resolve has generated a push for alternative management strategies since the recognition of the significant rates of spontaneous regression. This has led to the identification of several different options for non-ablative management in the hope of preventing the progression of the lesion or facilitating its regression. With the knowledge that the level of virus oncoprotein from integration is the key to cancer progression, a promising approach for non-ablative treatment has been to increase the immune clearance of viral infection. With the importance of minimizing additional morbidity and cost, a more decisive approach has been to simply monitor the disease with cytologic evaluations, colposcopy, and biopsy, and only treat at the time of potential progression becoming invasive cancer.
